At Case Management Cymru we are able to provide a highly professional and individualised specialist case management service to adults and children with:

We provide:

  • Immediate Needs Assessments – including clearly defined areas of need, actions to be taken and estimated costs
  • Coordination of care and therapy services
  • Support the client to achieve personal outcomes
  • Service plan development, implementation and monitoring
  • Regular reporting re budget and goals
  • Family support and education
  • Advocacy for clients
  • Welfare Benefit Assistance
  • Litigation related tasks including witness statements and court attendance
  • Recruitment, induction and training of support staff
  • Ongoing training, Supervision and Appraisal of support staff


We accept referrals from personal injury/medical negligence solicitors, Court of Protection Deputies or family members at any stage during or after the litigation process.  We understand the anxieties that people experience during this process and are able to support them throughout.

The first step is to provide us with enough information to allow us to identify Case Managers that have the skills and experience to meet the client’s needs.  Before a decision is made we encourage clients to meet potential Case Managers to ensure they are the ‘right fit’ for them.

The chosen Case Manager begins the initial assessment, working closely with the client and their family, drawing on the knowledge of other health and social care professionals involved.  Recommendations are made and a plan is designed in conjunction with the client and their family; looking at their priorities and the outcomes they want to achieve.
